Studies on informed consent to medical research conducted in low or middle-income settings have increased, including empirical investigations of consent to genetic research. We investigated voluntary participation and comprehension of informed consent among women involved in a genetic epidemiological study on breast cancer in an urban setting of Nigeria comparing women in the case and control groups.

We contrast person-centered categories with objective categories related to physics: consciousness vs. mechanism, observer vs. observed, agency vs. event causation. semantics vs. syntax, beliefs and desires vs. dispositions. How are these two sets of categories related? This talk will discuss just one such dichotomy: consciousness vs. mechanism. Two extreme views are dualism and reductionism. An intermediate view is emergence. Here, consciousness is part of the natural order (as against dualism), but consciousness is not definable only in terms of physical mass, (...) length, and time (as against reductionism). There are several detailed theories of emergence. One is based on the Great Chain of Being and on organic evolutionary hierarchy. The theory here is based instead on the concept of relational holism in quantum mechanics. The resulting brain model has two interacting systems: a computational system and a quantum system (a Bose-Einstein condensate), perhaps interacting via EEG waves. Thus, we need both person-centered and matter-centered categories to describe human beings. Some possible experimental tests are discussed. (shrink)

The paper deals with pp formulas in the language of reduced special groups, and the question of when the validity of a pp formula on each finite subspace of a space of orderings implies its global validity [18]. A large new class of pp formulas is introduced for which this is always the case, assuming the space of orderings in question has finite stability index. The paper also considers pp formulas of the special type $b\in \Pi _{i=1}^{n}\,D\langle 1,a_{i}\rangle $. ‘Smart’ devices are becoming increasingly ubiquitous. While these sophisticated machines are useful for various purposes, they sometimes evoke feelings of eeriness or discomfort that constitute uncanniness, a much-discussed phenomenon in robotics research. Adult participants (N = 115) rated the uncanniness of a hypothetical future smart speaker that was described as possessing the mental capacities for experience, agency, neither, or both. The novel condition prompting participants to attribute both agency and experience to the speaker filled an important theoretical gap in the (...) literature. Consistent with the mind perception hypothesis of uncanniness (MPH; Gray & Wegner, 2012), participants in the with-experience condition rated the device significantly higher in uncanniness than those in the control condition and the with-agency condition. Participants in the with-both (experience and agency) condition also rated the device higher in uncanniness than those in the control condition and the with-agency condition, although this latter difference only approached statistical significance. BackgroundThere is evidence that direct journal endorsement of reporting guidelines can lead to important improvements in the quality and reliability of the published research. However, over the last 20 years, there has been a proliferation of reporting guidelines for different study designs, making it impractical for a journal to explicitly endorse them all. The objective of this study was to investigate whether a decision tree tool made available during the submission process facilitates author identification of the relevant reporting guideline.MethodsThis was (...) a prospective 14-week before–after study across four speciality medical research journals. During the submission process, authors were prompted to follow the relevant reporting guideline from the EQUATOR Network and asked to confirm that they followed the guideline (‘before’). After 7 weeks, this prompt was updated to include a direct link to the decision-tree tool and an additional prompt for those authors who stated that ‘no guidelines were applicable’ (‘after’). For each article submitted, the authors’ response, what guideline they followed (if any) and what reporting guideline they should have followed (including none relevant) were recorded.ResultsOverall, 590 manuscripts were included in this analysis—300 in the before cohort and 290 in the after. There were relevant reporting guidelines for 75% of manuscripts in each group; STROBE was the most commonly applicable reporting guideline, relevant for 35% (n = 106) and 37% (n = 106) of manuscripts, respectively. Use of the tool was associated with an 8.4% improvement in the number of authors correctly identifying the relevant reporting guideline for their study (p < 0.0001), a 14% reduction in the number of authors incorrectly stating that there were no relevant reporting guidelines (p < 0.0001), and a 1.7% reduction in authors choosing a guideline (p = 0.10). However, the ‘after’ cohort also saw a significant increase in the number of authors stating that there were relevant reporting guidelines for their study, but not specifying which (34 vs 29%; p = 0.04).ConclusionThis study suggests that use of a decision-tree tool during submission of a manuscript is associated with improved author identification of the relevant reporting guidelines for their study type; however, the majority of authors still failed to correctly identify the relevant guidelines. L’ouvrage célèbre de Henry Sidwick, The Methods of Ethics, avait pour objet de trouver un fondement intuitionniste à l’utilitarisme. Les six conférences qu’il donna à la Société Métaphysique sur pratiquement le même sujet permettent de mieux comprendre le cheminement des idées de Sidgwick sur ce point à l’aune des changements religieux, scientifiques et politiques de la période. Sa critique de ce qui allait devenir l’Idéalisme britannique et les critiques dont il fut l’objet par un idéaliste tel que Bradley eurent des (...) incidences sur les révisions successives de cet ouvrage. En essayant de concilier utilitarisme et intuitionnisme – deux méthodes éthiques opposées – il tentait de défendre une forme d’utilitarisme nouveau, à savoir un utilitarisme dans lequel serait incorporé une forme de moralité discrète. Le fait qu’il échoua n’est pas important en soi car il tenta de trouver des solutions à ce qui lui paraissait être irréconciliable dans l’utilitarisme et les six conférences qu’il donna à la Société Métaphysique permettent d’en saisir toute la portée et de comprendre sa définition de l’utilitarisme. N. Katherine Hayles is known for breaking new ground at the intersection of the sciences and the humanities. In Unthought, she once again bridges disciplines by revealing how we think without thinking—how we use cognitive processes that are inaccessible to consciousness yet necessary for it to function. Marshalling fresh insights from neuroscience, cognitive science, cognitive biology, and literature, Hayles expands our understanding of cognition and demonstrates that it involves more than consciousness alone. Cognition, as Hayles defines it, is applicable not (...) only to nonconscious processes in humans but to all forms of life, including unicellular organisms and plants. This paper examines the US Atomic Energy Commission’s radioisotope distribution program, established in 1946, which employed the uranium piles built for the wartime bomb project to produce specific radioisotopes for use in scientific investigation and medical therapy. As soon as the program was announced, requests from researchers began pouring into the Commission’s office. During the first year of the program alone over 1000 radioisotope shipments were sent out. The numerous requests that came from scientists outside the United States, however, sparked (...) a political debate about whether the Commission should or even could export radioisotopes. This controversy manifested the tension between the aims of the Marshall Plan and growing US national security concerns after World War II. Proponents of international circulation of radioisotopes emphasized the political and scientific value of collaborating with European scientists, especially biomedical researchers. In the end, radioisotopes were shipped from the Commission’s Oak Ridge facility to many laboratories in England and continental Europe, where they were used in biochemical research on animals, plants, and microbes. However, the issue of radioisotope export continued to draw political fire in the United States, even after the establishment of national atomic energy facilities elsewhere. (shrink)

We use Homer and Sun Tzu as a background to better understand and reformulate confrontation, anger and violence in medicine, contrasting an unproductive ‘love of war’ with a productive ‘art of war’ or ‘art of strategy’. At first glance, it is a paradox that the healing art is not pacific, but riddled with militaristic language and practices. On closer inspection, we find good reasons for this cultural paradox yet regret its presence. Drawing on insights from Homer's The Iliad and The (...) Odyssey, we argue for better understanding of confrontation, anger, bullying, intimidation and violence in medicine in order to change the culture. For example, equating medicine with war is not a given condition of medicine but a convenient metaphor with historical origins and a historical trajectory. Other, non-martial metaphors, such as medicine as collaboration, may be more appropriate in an age of team-based care. Taking lessons from Homer, we suggest three key ways in which cold-hearted confrontation and anger in medicine can be transformed into productive, warm-hearted engagement: the transformation of angry impulse into reflection, moral courage and empathy. Thinking with Homer can offer an aesthetically and morally charged alternative to the current body of literature on topics, such as anger in doctors, and how this may be ‘managed’, without recourse to an instrumental economy where emotions are viewed as commodities, and emotional responses can be ‘trained’ through communication skills courses. Paul Marshall takes extrovertive mystical experience seriously by providing a metaphysical framework inspired by Plotinus and Leibniz that aims to interpret it non-reductively and to explain it persuasively. However praiseworthy Marshall's intentions, his account fails for a variety of reasons, among them an inability to establish convincingly why natural objects appear as transfigured and alive, characteristics frequently encountered in the reports of nature mystics. An alternative approach, rooted in contemporary pan-experientialist philosophy of mind, is able to take extrovertive (...) mysticism equally seriously while accounting more successfully for its pre-eminent features at a less extravagant metaphysical cost. An attempt to understand a problem fundamental to Plato's thought, and in a way which avoids the pitfalls of modern opinion, the author's expressed intention is to understand Plato as he understood himself. Though not indicated by the table of contents, her expository procedure is to present a sequential interpretation of the dialogues in relation to one another. As a result, the book does not relate in any systematic detail what might be the similarities and differences within the family of (...) Platonic terms related to desire; horme, himeros, boulesthai, eros, epithymia, thymos, and philia. The author's procedure might be readily understandable insofar as the dialogue form requires contextual interpretation. But it is puzzling insofar as her reflections on the hermeneutic problem posed by the dialogue are limited to a brief footnote remark : "Ce n'est pas seulement en vertu de son génie littéraire que la forme s'est imposée à l'oeuvre, au sens de l'oeuvre; sans le recours au moindre artifice, les Dlalogues sont une éducation en train de se faire.". In "The Law of Peoples" John Rawls casts his proposals as an argument against what he calls "political realism." Here, I contend that a certain version of "Christian political realism" survives Rawls's polemic against political realism sans phrase and that Rawls overstates his case against political realism writ large. Specifically, I argue that Rawls's dismissal of "empirical political realism" is underdetermined by the evidence he marshals in support of the dismissal and that his rejection of "normative political realism" is in (...) tension with his own normative concessions to political reality as expressed in "The Law of Peoples." That is, I contend that Rawls, himself, needs some form of political realism to render persuasive the full range of normative claims constituting the argument of that work. (shrink)

Philosophical aspects of belief and ancestor worship in Southern Vietnam.

Journalists in many Afiican countries have long been caught between differing ideals i n their relationship between press and government. Two models viefor dominance-the western, libertarian and development journalism models. This article uses Walzer's (1983) theory of distributive justice to illuminate the ethical significance of this debate. A t issue is political power. A case study of the 1996 proposed press law i n Kenya illustrates the ethical arguments mounted for each press model and how the arguments are marshaled not (...) necessarily for moral purposes but to gain political advantage. Finally, a viable third alternative avoids a false dilemma between the libertarian and development journalism models. Communitarianism preserves the independence from government SO central to the libertarian model while providing a basis for activist journalism. (shrink)

Thomas' remarks raise some interesting questions about the courage and cowardice of soldiers who happen to be fighting on the wrong side. These are interesting and complex questions of moral philosophy in their own right, and I did not feel that it was appropriate inCourageto dwell on them in too much detail. However, Thomas should have read more closely before declaring that “Walton does not even consider” cases of whether actions “on the part of the Axis soldiers would count as (...) courageous”. For there is a substantial discussion inCourageof the case of the actions of Field Marshal Erwin Rommell for which he was decorated in both World Wars. But Thomas is surely right that further discussion of other cases of this type will help to advance our understanding of problems in ethics of courage. (shrink)

Women are routinely subjected to penetrating surveillance during pregnancy. On the surface, this may appear to flow from a cultural commitment to protect babies – a cultural practice of “better safe than sorry” that is particularly vigilant given the vulnerability of fetuses and babies. In reality, pregnancy occasions incursions against human rights and well-being that would be anathema in other contexts. Our cultural practices concerning risk in pregnancy are infused with oppressive norms about women’s responsibility for pregnancy outcomes and the (...) demands of extreme self-sacrifice from women to protect their fetuses. Of particular concern is our culture’s willingness to enforce norms concerning risk during pregnancy using coercive measures including forced cesarean sections and criminal penalties for exposing fetuses to risk. This chapter will consider assaults on self-determination, bodily integrity and privacy inherent in such interventions, as well as the structural violence and “mangled pieties” that buttress such practices in our unjust society. Today's medical students are being confronted with ethical situations of far greater complexity than were their predecessors and yet the medical education system does little to prepare students for the ethical dilemmas which they inevitably face when entering the hospital environment. The following article addresses the issues surrounding a case where a patient has told a student in confidence of his plans to commit suicide. What should the student do? The only way for the student to prevent death is by (...) breaking confidentiality because the student has insufficient clinical experience to provide adequate guidance. However, this requires ignoring the patient's right to autonomy, a right enshrined in both case law and medical ethics. Clearly the student's ethical, moral and legal position must be carefully evaluated. (shrink)
